Not everything is thought through well enough. There are millions of moments of great design out there in the world, but there are also millions of horribly designed items. Designing something like a great product or a helpful piece of architecture isn't easy, but some designs are so bad that it's hard to imagine how they could have made something so awful.

There are tons of them out there, and some of them are downright facepalm-worthy. It could be an accessibility ramp that isn't so accessible or a normally innocent product that ended up being kind of inappropriate, or totally wrong; these moments of bad design are complete fails that made it to production somehow. Moments of poor design can be found all over, and sometimes they are exposed on the Internet for all to laugh at.
